The Project Intake Form is an important part of our process. It captures the most relevant information about your continuous improvement project and gives us a better understanding of the scope of your project and the support you would require from us.

The Project Charter is a one-page template to be updated by the project manager, using information captured in the Project Intake Form. The Project Charter will be used to present a new project summary information to the Governance Committee for evaluation.
